NOTE: In order to attend to service calls, this form MUST be used. Owner of property must submit this Service Request form. Verbal messages and telephones calls for service will not be acknowledged, aside from emergencies. **If you have an EMERGENCY please immediately call 800-481-9093 Ext 1**. Please keep in mind that maintenance of all components of the home is the responsibility of the homeowner. You may also submit this form by mail to S&S Homes of the Central Coast, Inc., 532 Camino Mercado, Arroyo Grande, CA, 93420 or by fax to 805-481-1083. You will receive an acknowledgment of this service request within 48 hours of receipt.
